The VA Center in St. Louis, MO has been cited for health and leadership problems again. In 2010, a nurse failed to monitor a patient who died one day after receiving dialysis. The 58 year old man became unresponsive during his five hour dialysis treatment, reports the Veterans Affairs Office of Inspector General. In the last two years, the VA was cited for violating sterilization rules in its dental and operating rooms. In February 2011, rust stains were found on some of the surgical equipment, which shut down the VA's operating rooms until they were cleared one month later and reopened.
